MEMBERSHIP APPLICATION
If you are interested in membership at Calvary Church you will need to fill in a membership application form and bring it in to the church office. The form will be forwarded to the church board for review. Someone from the church office or board will be in contact with you about your application.
Application forms are available through the church office.

BAPTISM APPLICATION
At Calvary Church we believe in believer's baptism by total immersion. If you are a follower of Christ and have never been baptized in water we encourage you to take this step of obedience. Please download the application form, fill it in and bring that to the church office or one of the pastors. Someone will be in contact with you to discuss taking the next steps towards water baptism.

BUILDING USE FORM
If you are interested in using Calvary Church's facilities for an event please download the attached form and bring it in to the church office. Our facilities are used for weddings, funerals and other events.
